OSHA Respirator Medical Clearance

Welcome to the Oshamedcert Medical Evaluation and Certificate System.

You can now complete your respirator medical evaluation questionnaire online and be certified by a doctor. Your responses will be handled in a strictly confidential manner. Once you've begun working through the questionnaire please finish. If you have any problems or questions, please contact your Respiratory Protection Program Administrator

Most people qualify just by completing this evaluation and do not need additional follow-up with a physician or other licensed health care professional.

  •           Who needs to be Certified?

This certification is for employees that are required to use one or all of the following:

    • disposable respirators with N, R, or P ratings
    • half mask air-purifying or atmosphere-supplying respirators
    • full facepiece air-purifying or atmosphere-supplying respirators

This questionnaire will take approximately 5-10 minutes to complete. To begin, it will be necessary to log on with the Access Code supplied by your Respiratory Protection Program Administrator.

Note: Air-purifying respirators include PAPRs
Atmosphere-supplying respirators include SARs and SCBAs
